Clearpoint began with a simple frustration: patients were repeating their story at every door. We built one organization where primary, specialty, and diagnostic teams share the same record and the same plan.

To give every patient in the Denver metro area a medical home where care is coordinated, explained in plain language, and easy to access when it matters.
How we practice
Providers share one chart, meet weekly on complex cases, and write care plans with patients rather than for them. Visits are scheduled with enough time to finish the conversation.
